When is this form required?

  • Passengers whose fitness to fly is in doubt due to recent illness or surgery.
  • Passengers requiring medical oxygen or the use of specialized medical equipment on board.
  • Expectant mothers with complicated pregnancies or those beyond specific gestations.
  • Any passenger whose medical condition may be aggravated by flight conditions.
